This position is listed on behalf of a partner company, who manages all applications and next steps. Our partner is looking for a Account Manager, ACOs & Medicare Supplement (MediGap) Health Plans based in United States. This high-ownership role manages strategic healthcare partnerships across ACOs and Medicare Supplement health plans. You will serve as the primary relationship owner, building trust with clinical, operational, carrier, and executive stakeholders. The position combines account management, program performance, provider enablement, enrollment growth, and operational coordination. You will help partners leverage the CMS GUIDE Model to improve engagement and outcomes for people and families affected by dementia. Success requires translating complex healthcare workflows and performance data into practical actions that drive adoption and measurable results. You will work cross-functionally with implementation, care coordination, coaching, operations, marketing, and engagement teams. This is a remote-first opportunity in a fast-moving environment where your work can directly influence how innovative dementia care programs scale nationwide.
