ABOUT THE ROLE This is a hands-on AI engineering role at a healthtech company in the medical tourism space, where you will own the patient-facing AI conversation loop end-to-end. You will sit at the intersection of infrastructure and empathy, building robust agent systems that directly shape real patient outcomes. The work matters: when the AI stumbles, you fix it, and when patients need a human touch, you provide it. WHAT YOU'LL DO

  • Diagnose why AI conversations with patients fail and ship code fixes to prevent recurrence.
  • Build and maintain agent infrastructure, including tool integrations, evaluation frameworks, and retrieval systems.
  • Automate recurring manual workflows on the clinic side, eliminating toil through code.
  • Handle patient-facing situations with empathy when the AI falls short, turning those moments into product insights.
  • Own systems end-to-end with minimal oversight, making architectural and implementation decisions independently. WHAT WE'RE LOOKING FOR
  • 2+ years of demonstrated experience building and shipping LLM or agent systems end-to-end (professional, personal projects, research, or open-source all count equally).
  • Hands-on experience with agent infrastructure: evals, RAG/retrieval systems, and tool integrations.
  • Proficiency in Python or an equivalent language for building agent infrastructure.
  • Strong computer science and mathematics fundamentals; elite academic signals such as top-tier university backgrounds, published research, or mathematics competition experience are a strong plus.
  • Automation instinct: you spot recurring manual workflows and proactively eliminate them through code.
  • Ability to diagnose and fix failures in production LLM/agent systems.
  • Familiarity with vector databases, embedding systems, or semantic search is a plus.
  • Experience with prompt engineering, fine-tuning, or conversational AI evaluation is a plus.
  • Patient-facing warmth and empathy; comfort handling difficult conversations with care.
  • Startup autonomy: comfortable owning systems independently in fast-moving environments.
  • Existing US work authorization required; no visa sponsorship available. COMPENSATION & BENEFITS Base salary: $150,000 to $200,000 USD annually. Visa sponsorship is not available; existing US work authorization is required. LOCATION This role is fully on-site, five days per week, in San Francisco, California, United States. No remote exceptions.
