Job Title: Clinical Anticoagulation Pharmacist Banding: Band 7 Location: Chertsey Surrey Start Date: ASAP Duration: Ongoing Hourly Rate: £30 - £32 Working Hours: Monday to Friday, Full-Time Job Overview Pulse is currently seeking an experienced and motivated Band 7 Clinical Anticoagulation Pharmacist to provide specialist locum support to the pharmacy team in Chertsey Surrey. This is an excellent opportunity for a clinical pharmacist with experience in anticoagulation services to play a key role in delivering safe, effective, and patient-centred care within a busy NHS hospital setting. The successful candidate will work closely with multidisciplinary teams to optimise anticoagulation therapy, support medicines management initiatives, and ensure patients receive the highest standard of pharmaceutical care. Key Responsibilities Provide specialist clinical pharmacy support for anticoagulation services. Review and optimise anticoagulation therapy to ensure safe and effective patient care. Provide expert pharmaceutical advice to medical, nursing, and allied healthcare professionals. Undertake clinical medication reviews and medicines reconciliation activities. Monitor patients receiving anticoagulant therapy and support dose adjustments where appropriate. Contribute to the development and implementation of anticoagulation-related guidelines and protocols. Support patient education and counselling regarding anticoagulant medicines. Participate in audit, governance, and quality improvement initiatives relating to anticoagulation services. Qualifications and Experience Be registered with the General Pharmaceutical Council (GPhC). Have previous NHS hospital pharmacy experience at Band 7 level or equivalent. Demonstrate experience working within anticoagulation services or managing anticoagulant therapies. Possess strong clinical knowledge of anticoagulant medicines and associated monitoring requirements. Be confident working independently and within multidisciplinary teams.
