This position is listed on behalf of a partner company, who manages all applications and next steps. Our partner is looking for a Healthcare - Managed Care Community Connector based in United States. This fully remote role supports members with complex healthcare and social needs by helping them navigate services, benefits, and community resources. You will engage directly with members to coordinate appointments, address barriers to care, and help close gaps in healthcare. The position supports an integrated approach that may include behavioral health and long-term care services. You will work with vulnerable and underserved populations across varied health, economic, and educational circumstances. The role combines member advocacy, outreach, care coordination, research, and connection to essential community resources. You will collaborate with care managers and multidisciplinary teams to help members progress toward appropriate and cost-effective care. This is an opportunity to make a meaningful difference by helping individuals access the healthcare and social support they need.

Identify members who meet specific outreach and support criteria using available platforms, systems, and internal data. Conduct telephone outreach and member visits to assist with provider appointments, address concerns, and close gaps in care. Serve as a community-based member advocate and resource by using knowledge of available healthcare and community services. Support the Healthcare Services team through non-clinical activities, including engagement with members in appropriate community settings when required. Help members navigate and maximize their health plan benefits, including scheduling appointments, arranging transportation, obtaining prescriptions, and following up on missed appointments. Assess barriers to care and provide care coordination and practical assistance to help members address identified needs. Connect members with social and community resources related to housing, food, employment, and other essential services. Provide outreach and support to disconnected members and individuals with special healthcare or social needs. Conduct research to locate members who have been difficult to contact by reviewing internal databases and communicating with providers or caregivers when appropriate. Participate in ongoing and project-based outreach initiatives through telephone and, when required, face-to-face engagement. Promote integration of services, including behavioral health and long-term care, across the member's care journey. Help members maintain Medicaid eligibility and identify other appropriate financial resources. Collaborate with licensed care managers and leadership as needed to support member outcomes. Requirements: Have a high school diploma or GED. Have at least 1 year of experience working with underserved or special-needs populations across varied health, economic, and educational circumstances. Demonstrate strong communication and interpersonal skills for engaging members by telephone and through other outreach channels. Be able to understand member needs, identify barriers to care, and connect individuals with appropriate healthcare and community resources. Demonstrate empathy, advocacy, organization, and the ability to work effectively with vulnerable populations. Be comfortable using databases, platforms, and other systems to identify members, conduct research, and support outreach activities. Be able to collaborate effectively with care managers, leadership, providers, caregivers, and multidisciplinary teams. Demonstrate the ability to work independently in a fully remote environment while managing member outreach and follow-up activities. Benefits: Fully remote work within the United States, with preference for candidates working Central Standard Time while remaining open to all U.S. time zones. Monday–Friday schedule from 8:00 a.m. to 5:00 p.m. CST. Required home-office equipment includes dual monitors, a docking station, connecting cables, headset, mouse, and keyboard. The initial engagement is expected to have potential for extension beyond 6 months and/or conversion to a full-time employee position. The role provides the opportunity to make a direct impact by helping underserved and high-need members access healthcare, behavioral health, and essential community resources.
