You will undertake comprehensive assessments, develop and implement care plans, provide ongoing case management, and work closely with individuals and wh&#257nau to support positive mental health outcomes.This is a varied and autonomous role where you will manage a caseload, participate in duty services, and collaborate with a multidisciplinary team to provide coordinated care.
