The Palliative Care Nurse Practitioner serves as a clinical leader in the community-based palliative care program. The NP provides advanced assessment, diagnosis, and management of patients with serious illness across care settings. This role emphasizes patient- and family-centered care, expert communication, and coordination with the interdisciplinary team to improve quality of life, align treatment with patient goals, and ensure continuity of care.
ESSENTIAL JOB FUNCTIONS:

Conduct comprehensive patient assessments, focusing on physical, psychosocial, and spiritual needs.

Develop and implement individualized care plans in collaboration with patients, families, and the interdisciplinary team.

Facilitate advanced care planning and goals-of-care discussions with patients, families, and healthcare providers.

Prescribe medications, treatments, and interventions consistent with licensure and scope of practice.

Provide clinical leadership and mentoring to the interdisciplinary team.

Ensure compliance with Joint Commission, state, and federal regulations.

Participate in quality improvement initiatives and program evaluation.

Travel required to patient locations throughout the New York metro area.

Works within legal, ethical practice standards to his/her position as well as Calvary’s Code of Conduct. Comply with appropriate policies and procedures and brings potential compliance matters to supervisor or compliance officer.
Requirements
QUALIFICATIONS:

Current NYS Registered Nurse license.

Current NYS Nurse Practitioners license.
-
Master’s or Doctoral degree in Nursing; certification as an Advanced Practice Nurse in Palliative Care or related specialty preferred.

Minimum of two years of clinical experience as a Nurse Practitioner in palliative care, hospice, oncology, geriatrics, or related field preferred.

New York State Driver’s License preferred.

Demonstrated expertise in complex symptom management, advance care planning, and interdisciplinary collaboration.

Exhibits an ability and attitude relative to the spirit of caring and tradition at Calvary Hospital as required by specific job duties and functions.
