Opportunity Information

Palo Alto Foundation Medical Group is seeking a part-time, certified Nurse Practitioner or Physician Assistant in Palliative Care.

Location: Palo Alto, CA

Highlights:

Responsibilities:

  • Managing health problems and coordinating health care for the patient in accordance with State Federal Rules and Regulations

  • Provide comprehensive, coordinated, and compassionate care for patients and families facing serious illness

  • Formulates a comprehensive care plan, patient education, and ongoing evaluation in collaboration with the palliative care interdisciplinary team for palliative care patients in the home, office and SNF settings

  • Prescribes and administers medications/immunizations including controlled substances to the extent delegated and licensed, and in collaboration with palliative care and/or referring physician

Qualifications

Requirements:

  • 6 months related clinical experience in Oncology, Geriatrics, Hospice or Palliative Care

  • Current California State NP or PA License

  • CAPA Controlled Substance Certification (for Physician Assistants only)

  • Current Nurse Practitioner Furnishing Number (for Nurse Practitioners only)

  • Nurse Practitioner furnishing number of NP’s

  • Active DEA registration
