Summary: Assist in the collection, aggregation and analysis of data for Home Care and Hospice QA&I monitors. Responsible for data entry for intra-departmental monitoring as well as regulatory monitoring. Assists the Assistant Director of Regulatory Compliance and the Regulatory Compliance Coordinator in all educational and compliance activities.

Location: Bronx Campus

Status: Full - Time

Salary Range: 116,983.73 - 143,483.74/ annually

ESSENTIAL JOB FUNCTIONS:

  • Participate in the collection of data generated for the Quality Assessment & Performance Improvement program, regulatory and compliance for Calvary Home Care and Hospice.
  • Develop and format data collection tools as directed.
  • Maintain accurate and confidential records of data collected.
  • Prepares Hospice CAHPS mailing list monthly prior to submission to Press Ganey.
  • Calculate response rates for various surveys and data collection processes.
  • Coordinates the timely and accurate implementation of telephone surveys; collate responses.
  • Enters survey/QA & PI data onto appropriate computer program.
  • Prepares mailings and meeting arrangements and takes minutes for Calvary @ Home
  • QA & I and Professional Advisory Committees and Performance Improvement meetings.
  • Maintains accurate education records on each employee within Calvary @ Home.
  • Collects Medicare D information and notifies appropriate Medicare D Plan upon admission and discharge for Medicare Hospice patients.

Requirements

  • Bachelor’s Degree required.
  • Minimum 2 years data entry experience required with accurate typing ability.
  • Knowledge of Microsoft Office (Word, Excel spreadsheets).
  • Knowledge of graphic display of information.
  • Attention to detail and follow-up; proven organization skills.
  • Exhibits an ability and attitude relative to the spirit of caring and tradition at Calvary Hospital as required by specific job duties and functions.
