Social Worker

This position is listed on behalf of a partner company, who manages all applications and next steps. Our partner is looking for a Social Worker based in the United States. This fully remote role provides behavioral health and social support to members facing complex health and social needs. You’ll assess psychosocial circumstances, identify social determinants of health, and help remove barriers to care. The role combines telephonic outreach, supportive counseling, care coordination, and community resource navigation. You’ll work closely with an interdisciplinary clinical care team to support comprehensive member outcomes. The position offers an opportunity to make a meaningful difference by connecting individuals and families with practical resources and services. Success requires strong communication, cultural awareness, organization, and the ability to work independently. Limited travel may be required based on business needs.

Conduct behavioral health assessments to identify psychosocial needs, social determinants of health, and barriers to care. Communicate assessment findings and relevant member needs to the Clinical Care Team to support individualized care planning. Conduct telephonic outreach to engage eligible members and encourage participation in care management programs. Provide actionable interventions and supportive counseling to members and families addressing needs such as housing, finances, utilities, transportation, and mental health. Connect members with appropriate community health, human services, and wrap-around resources. Encourage adherence to provider treatment plans and established care team plans. Collaborate with external agencies and community organizations to support continuity of care. Maintain accurate and timely documentation in care management systems. Participate in team meetings, individual meetings with leadership, and ongoing in-service training. Meet individual performance metrics established by leadership and complete other related responsibilities as required. Requirements Master’s degree in Social Work is required. An active, unrestricted state-issued LCSW license is highly preferred. At least 3 years of experience in a healthcare or human services agency setting. Experience working in a managed care environment. Knowledge of community-based resources and services. Experience working with renal health populations is preferred. Strong interpersonal skills with the ability to motivate and engage members and other stakeholders. Excellent written and verbal communication skills. Strong cultural awareness and the ability to work effectively with diverse populations. Ability to independently identify problems, clarify information, organize findings, recognize trends, and determine when additional information is needed. Proficiency with Microsoft Office applications, including Outlook, Word, Excel, and PowerPoint. Ability to work effectively as part of an interdisciplinary clinical team while managing responsibilities independently. Benefits 100% remote work arrangement within the United States.
