This position is listed on behalf of a partner company, who manages all applications and next steps. Our partner is looking for a Supervisor, Transitions of Care Coordinator based in United States. This role provides frontline leadership for a virtual team supporting patients as they transition from hospitals or post-acute settings back to home. You will guide care coordinators who help patients access essential services, appointments, transportation, supplies, and durable medical equipment. The position combines people leadership with hands-on patient support, with approximately 30% of time dedicated to direct transitions-of-care activities. You will help strengthen workflows, develop job aids, monitor performance, and coach team members toward high-quality outcomes. The role also offers the opportunity to contribute to program development, cross-functional initiatives, and evolving care models. Success requires an organized, empathetic, technology-savvy leader who can balance operational performance with a strong patient-centered approach.
