Responsibilities: As a Band 4 Locum Therapy Assistant you will be responsible for working within our Integrated Transfer of Care Team, providing essential support for patient discharges, conducting thorough assessments in the community, and ensuring the appropriate provision of equipment. Your role will involve traveling within the community to visit patients, requiring a valid driver’s license. You will work closely with other healthcare professionals to deliver comprehensive care and support, ensuring smooth transitions for patients from hospital to home.

Qualifications and Experiences:

  • Experience in a healthcare setting, preferably in community care.

  • Valid driver’s license and ability to travel within the community.

  • Strong communication and interpersonal skills.
