Cardiovascular Clinical Guide

This position is listed on behalf of a partner company, who manages all applications and next steps. Our partner is looking for a Cardiovascular Clinical Guide based in the United States. This fully remote role supports members enrolled in a specialized virtual hypertension care program. You will guide members through their cardiovascular health journey using education, coaching, remote monitoring, and care coordination. The role combines clinical assessment, lifestyle coaching, patient engagement, and collaboration with multidisciplinary care teams. You will help members understand their health conditions, establish achievable goals, and develop sustainable habits. You will also coordinate follow-up care and connect members with appropriate clinical and community resources. Success in this position requires strong communication, empathy, organization, and experience supporting people with chronic conditions. The role offers an opportunity to contribute to accessible, home-based care while helping members make meaningful improvements in their cardiovascular health.

Screen members for eligibility in the hypertension care program and support enrollment throughout their care journey. Provide blood pressure monitors, assess members’ measurement techniques, and reinforce accurate self-monitoring practices. Conduct comprehensive hypertension assessments covering medical history, medications, adherence patterns, lifestyle habits, and relevant health behaviors. Deliver lifestyle and nutrition counseling focused on sodium and alcohol reduction, heart-healthy eating, portion control, weight management, physical activity, and smoking cessation. Identify barriers to healthy behaviors and coach members toward realistic, achievable, and sustainable health goals. Apply motivational interviewing techniques to encourage positive and lasting behavioral changes. Serve as a primary point of contact for members throughout their participation in the program. Conduct outreach calls and follow-up interactions as needed to support engagement and continuity of care. Schedule follow-up appointments with clinical providers, the member’s primary care provider, or other appropriate resources. Coordinate care with multidisciplinary teams, including specialty care nurse practitioners, clinical pharmacists, social workers, behavioral health professionals, community support teams, clinical associates, and outreach teams. Synchronize care plans and communicate relevant information with primary care and community-based providers. Connect members with appropriate internal resources to resolve clinical, logistical, or care-coordination needs. Maintain accurate and timely documentation of all member interactions and care activities. Identify and address applicable quality-measure gaps during member interactions to support program and care-quality objectives. Requirements Experience working as a Medical Assistant or in a clinical or healthcare setting. At least 2 years of experience supporting patients through longitudinal chronic disease management, such as primary care or specialty care. Strong patient coaching and engagement skills, with the ability to build trust and communicate health information clearly. Experience assessing patient needs, identifying barriers to care, and supporting behavior change. Strong organizational skills and attention to detail for managing documentation, follow-ups, and multiple member needs. Ability to collaborate effectively with multidisciplinary clinical and support teams. Strong verbal and written communication skills and a compassionate, member-focused approach. Ability to work independently in a remote environment while maintaining consistent communication and follow-through. Spanish-English bilingual proficiency is highly desirable. Experience working with Medicare patients is preferred. Comfort working with remote monitoring tools and virtual care workflows is beneficial. Availability to work one of the designated schedules aligned with Eastern, Central, Mountain, or Pacific time zones. Benefits Pay range: $21–$30 per hour, with actual compensation dependent on relevant qualifications, experience, skills, education, certifications, and work location.
