About the RoleOur Home Team is available to provide supported discharge of patients working across a range of settings including the Emergency Department, in-patient areas and community, as well as hospital avoidance. They are focused on delivering one car up the driveway, one assessment and one plan of care.This is a fantastic opportunity for a Registered Nurse interested in working as part of an Inter-professional team.Tasks for this role include but are not limited to:Support the early discharge of appropriate patients from Southland HospitalMonitor their recovery, ensure everyone is able to maintain their independence in their own homeWorking in a comprehensive, integrated service that are responsive to patient and service needsThis role covers a Monday to Sunday service between the hours of 8 am to 5 pm and also includes public holiday work. About youThis role requires a nurse who is adaptive to change, resourceful, flexible to service needs and future development and a genuine passion for working in the community.Our ideal candidate will have:A current APC and be a Registered Nurse with Nursing Council of New ZealandAt least two years post registration nursing experience in New ZealandA clean full New Zealand Drivers licenceComprehensive clinical assessment and problem-solving skills.Ability to work autonomously and collaboratively within the inter-professional team.Ability to direct and delegate to Allied Health Assistants.Excellent communication skills to liaise with health professionals, both within the in-patient services and in the community setting.Recent experience of working with the older adults in acute, rehabilitation or community settingsTo be successful in the role you will have a strong commitment to your ongoing professional development, and it is expected that you will have / or be committed to working towards a post-graduate pathway. Competence in assessment of complex patient needs is desired.
