As a Transition Support Coordinator, you will be responsible for providing face-to-face and/or virtual Specialized Transition Services to members seeking services within community settings. You will work collaboratively with Care Managers to facilitate member transition and community living, housing navigation, resources, and funding options. You may also complete review of person-centered care plans regarding services needed based on set guidelines.
The Illinois Tailored Care Management Program helps individuals with complex health and social needs transition from nursing facilities and Specialized Mental Health Rehabilitation Facilities (SMHRFs) to community-based living. Through person-centered care management and coordinated support services, the program empowers members to live more independently and thrive in their communities.
Performs face-to-face and/or virtual specialized transition services for persons with disabilities, behavioral health needs, and complex medical care needs Collaborate with care management team, health care providers, and ancillary service providers to implement the appropriate care plan support Documents transition support activity results for the individual member status and update care plan as needed. Provides ongoing education May communicate with all health care providers to ensure appropriateness of care Performs miscellaneous duties as assigned
Four-year degree in a health care or related field and/or relevant experience Position requires a valid driver’s license and up to 95% local travel
Case Management experience preferred Bilingual in Englis and Spanish a plus
